We partner with Anthem to administer your medical coverage. Employees working 30 or more hours per week are eligible for medical coverage. Anthem provides extensive online resources including cost comparison tools, various wellness tools, on call nurse line and more. As an eligible employee of DSB Technologies, you may choose the benefit coverage that fits you and your family’s need. You have a choice between three medical plans administered by Anthem with two network options. Employees residing in Wisconsin will utilize the Blue Preferred Network while employee residing outside of Wisconsin will utilize the Blue Access Network.

Monthly Non-Tobacco Rates Monthly Tobacco Rates The contributions above reward employees who do not utilize tobacco products. A tobacco-use question will be asked in ADP during enrollment and will determine which contributions you qualify for. You can also qualify for the Non-Tobacco Rates if you complete a smoking cessation program. After the completion of the program, you will be refunded the additional surcharge amount you contributed, back to January 1.
